How do you write an SBAR handover?

Situation, Background, Assessment, Recommendation — in that order, each part short. The point is that the person receiving it hears the same things in the same order every time.

• Situation: who, and what is happening right now.
• Background: what is relevant from before — conditions, medicines, the course so far.
• Assessment: your professional assessment, in your own words.
• Recommendation: what you are asking for or propose.
• Keep each part short. Detail belongs in the record, not in the handover.

How it works

You dictate the report however it comes out, and it is sorted into the four parts with values and times kept where you said them. The assessment stays yours — nothing clinical is added.
